Roles & Responsibilities

• Provide compassionate, hands-on care to residents, supporting dignity and comfort at every interaction
• Assist with activities of daily living including bathing, dressing, grooming, toileting, and mobility
• Monitor and record vital signs and resident responses, reporting changes promptly to nursing staff
• Follow individualized care plans and physician orders to ensure consistent, high-quality care
• Document care accurately and timely in the resident record to maintain clear communication
• Promote a safe, clean environment by following infection control and safety protocols
• Collaborate with nurses, therapists, and support staff to deliver coordinated care

Qualifications

• Current Licensed Nursing Assistant (LNA) certification (VT or NH as applicable)
• Completion of an approved nursing assistant program and state registry in good standing
• Current basic life support (BLS/CPR) preferred or willingness to obtain
• Ability to pass background check and meet facility required health screenings and immunizations
• Strong communication skills and a caring, patient-centered approach
• Reliable, punctual, and able to work evening shifts as scheduled

Education

• High school diploma or GED preferred
• Completion of a state-approved nursing assistant program and current LNA certification

Knowledge & Skills

• Competence with basic nursing assistant duties: ADLs, transfers, feeding assistance, and toileting
• Ability to take and document vital signs accurately
• Familiarity with infection control best practices and safety procedures
• Good charting and documentation habits to support clear care transitions
• Team player attitude, strong communication skills, and empathy for residents and families
• Comfortable working in long-term care or skilled nursing environments
