Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Help patients with bathing, dressing, grooming, toileting, and feeding in a respectful manner
• Monitor and record vital signs and report any changes to the nursing team
• Accurately document care and maintain clear patient records
• Support safe transfer and mobility techniques to prevent falls and promote independence
• Follow infection control practices and maintain a clean, safe environment
• Communicate effectively with nurses, therapists, patients, and families to coordinate care
• Contribute to a positive team environment and help create meaningful patient experiences

Qualifications

• Current Licensed Nursing Assistant (LNA) certification valid for practice in NH (and VT if applicable)
• Active state registry status and ability to pass facility onboarding requirements
• Comfortable working 12.5-hour day shifts (7:00 AM - 7:30 PM) with a 30-minute meal break
• Previous experience in long-term care or acute care is a plus but new graduates are welcome
• Basic CPR certification preferred or willingness to obtain upon hire
• Reliable, professional, and patient-focused with good communication skills

Education

• High school diploma or equivalent
• Completion of a state-approved Nursing Assistant training program (LNA)

Knowledge & Skills

• Strong patient care fundamentals and hands-on assistance skills
• Familiarity with infection prevention and basic safety protocols
• Accurate vital sign measurement and documentation
• Good interpersonal skills for interacting with patients and families
• Ability to follow care plans and work collaboratively within the care team
• Time management and ability to prioritize patient needs during busy shifts
