Roles & Responsibilities
- Provide compassionate hands-on care and assistance with activities of daily living
- Monitor patients and report changes in condition to the clinical team
- Document care accurately and promptly in the patient record
- Support a clean, safe, and comfortable environment for residents
- Collaborate with nurses and therapists to follow individualized care plans
- Engage families with respect and clear communication to support patient well-being
Qualifications
- Current Licensed Nursing Assistant (LNA) credential (VT or NH region indicated)
- Comfortable working 12+ hour day shifts (7:00 AM - 7:30 PM) on a per diem basis
- Reliable, patient-focused, and able to follow care plans and facility policies
- Strong communication and teamwork skills
Education
- Completion of a state-approved Nursing Assistant program or equivalent
Knowledge & Skills
- Basic patient care and transfers, bathing, feeding, and toileting assistance
- Vital signs and basic clinical observations
- Accurate documentation and charting practices
- Infection control and safety protocols
- Clear communication with patients, families, and the care team
- Time management for fast-paced shift work
