Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living
• Assist with safe patient transfers, positioning, and mobility support
• Measure and record vital signs and report changes to nursing staff promptly
• Maintain accurate, timely documentation of care and observations
• Support infection control practices and maintain a clean, safe environment
• Work collaboratively with the care team to meet patient-centered goals
• Help create a comforting atmosphere for patients and their families during overnight hours

Qualifications

• Active Licensed Nursing Assistant (LNA) credential — valid in New Hampshire (VT licensure a plus if covering Vermont)
• Completion of a state-approved nursing assistant program
• Current CPR/BLS certification preferred
• Demonstrated ability to provide compassionate, patient-focused care
• Strong communication skills and ability to work well within a team
• Previous experience in long-term care or similar setting preferred

Education

• High school diploma or equivalent
• Completion of an accredited nursing assistant (LNA/CNA) program

Knowledge & Skills

• Knowledge of ADLs and basic patient care procedures
• Ability to take and record vital signs accurately
• Familiarity with infection control and safety protocols
• Good documentation and time-management skills
• Comfortable with night-shift workflow and working independently
• Teamwork, empathy, and strong interpersonal skills
