Roles & Responsibilities

• Provide compassionate, hands-on personal care to residents and patients
• Assist with activities of daily living including bathing, grooming, dressing, and toileting
• Monitor and accurately record vital signs and observations
• Support safe mobility and transfers, using proper body mechanics
• Communicate changes in patient condition promptly to the nursing team
• Document care and interventions clearly and professionally in resident charts
• Collaborate with interdisciplinary staff to deliver consistent, person-centered care

Qualifications

• Valid LNA (Licensed Nursing Assistant) credential — NH or VT licensure acceptable
• Completion of a state-approved nursing assistant training program
• Current basic life support (BLS) or CPR certification preferred or required per facility
• Willingness to complete background checks and facility onboarding requirements
• Strong interpersonal skills and a passion for direct patient care

Education

• Completion of state-approved nursing assistant program
• High school diploma or equivalent preferred

Knowledge & Skills

• Comfortable with hands-on patient care and ADL assistance
• Ability to take and document vital signs accurately
• Basic infection control and safety practices
• Good communication skills with patients, families, and team members
• Reliable time management and attention to detail in charting and handoffs
• Team-oriented mindset and adaptability in a fast-paced clinical setting
