Location: Lebanon, New Hampshire
Contract Type: Per-Diem
Shift Type: Per diem, 7:00 PM - 7:30 AM NOC (overnight; schedule varies based on staffing needs)
Hours per Week: 36
Candidate Pay Rate: $23 per hour

Roles & Responsibilities
- Provide compassionate, hands-on personal care and assistance with activities of daily living
- Monitor and report patient status and changes to the nursing team
- Take and record vital signs accurately and promptly
- Maintain clear, timely documentation in the patient chart
- Support a safe, clean, and comfortable environment for residents
- Collaborate with nurses and interdisciplinary staff to carry out care plans
- Assist with patient transfers and mobility while following safe lift procedures
Qualifications
- Current Licensed Nursing Assistant (LNA) credential
- Valid license in New Hampshire or Vermont as appropriate for assignment, or ability to obtain
- Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Strong communication skills and a team-oriented mindset
- Reliable, punctual, and comfortable working overnight shifts
- Previous experience in long-term care or similar setting is a plus; new graduates welcome
Education
- High school diploma or equivalent
- Completion of an approved Nursing Assistant training program
Knowledge & Skills
- Skilled in ADLs: bathing, dressing, feeding, toileting, grooming
- Proficient with taking and documenting vital signs
- Infection control and safe patient handling practices
- Basic charting and electronic documentation familiarity
- Excellent interpersonal skills and resident-focused care
- Ability to prioritize tasks in a fast-paced environment
