Roles & Responsibilities

• Provide compassionate, hands-on personal care and assistance with activities of daily living (bathing, dressing, toileting)
• Monitor and record vital signs, intake/output, and report changes to the nursing team promptly
• Assist with safe patient transfers and mobility, following facility safety protocols
• Maintain clear, accurate documentation and communicate effectively during shift handoffs
• Support a clean, safe environment by following infection control and safety procedures
• Work as a collaborative team member to deliver person-centered care and a positive patient experience

Qualifications

• Current Licensed Nursing Assistant (LNA) certification (required)
• Eligible for or current NH LNA licensure to work in Lebanon, NH
• Comfortable working a long day shift (7:00 AM - 7:30 PM) on a per-diem basis
• CPR/BLS certification preferred or willingness to obtain
• Up-to-date immunizations and TB screening as required by facility policy
• Ability to pass background check and drug screen, and reliable transportation to the facility

Education

• High school diploma or GED and completion of a state-approved LNA training program

Knowledge & Skills

• Strong interpersonal and communication skills with patients and team members
• Basic clinical skills: taking vital signs, completing ADL care, assisting with mobility
• Good documentation and time-management habits
• Understanding of infection prevention and patient safety practices
• Compassionate bedside manner and the ability to remain calm under pressure
• Team-oriented approach with a willingness to learn and take direction
