Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living
• Take and record vital signs accurately and communicate changes to the nursing team
• Assist with mobility, transfers, and safe patient handling to promote independence
• Perform timely and accurate documentation in the patient record
• Support infection control practices and maintain a clean, safe environment
• Collaborate with nurses and interdisciplinary team members to deliver person-centered care
• Respond to patient needs with empathy, professionalism, and a sense of urgency when required

Qualifications

• Current LNA / Licensed Nursing Assistant certification (NH or VT) — must be valid and in good standing
• Basic Life Support (BLS) / CPR certification preferred or able to obtain promptly
• Ability to pass background check, drug screening, and meet facility onboarding requirements
• Strong communication skills and a team-oriented attitude
• Previous experience in long-term care, skilled nursing, or similar settings is a plus

Education

• High school diploma or GED and completion of an accredited nursing assistant program
• Active LNA / CNA certification (state-specific as required)

Knowledge & Skills

• Skilled in patient care basics: bathing, feeding, toileting, and mobility assistance
• Accurate vital signs measurement and basic clinical observations
• Familiarity with infection prevention and proper PPE use
• Comfortable with documentation and electronic charting systems
• Good time management, prioritization, and teamwork under busy conditions
• Compassionate bedside manner and strong interpersonal skills
