Roles & Responsibilities

• Provide compassionate, hands-on personal care and assistance with activities of daily living
• Monitor and document resident condition and communicate changes to the nursing team
• Assist with safe transfers, ambulation, and range of motion activities
• Support residents at mealtimes, with grooming, and with bathroom needs while preserving dignity
• Follow infection control and safety protocols to maintain a clean, secure environment
• Collaborate with nurses and other caregivers to deliver person-centered care plans

Qualifications

• Current Licensed Nursing Assistant (LNA) certification; VT or NH licensure noted in posting
• Comfortable working overnight shifts and adapting to changing per diem schedules
• Strong communication skills and the ability to work well within a multidisciplinary team
• Current CPR/BLS certification preferred or willing to obtain
• Reliable, compassionate, and committed to resident-centered care

Education

• High school diploma or equivalent
• Completion of a state-approved nursing assistant training program and current LNA certification

Knowledge & Skills

• Practical experience with ADLs, transfers, and ambulation support
• Basic clinical observation and documentation skills
• Knowledge of infection prevention and safe patient handling
• Ability to follow care plans and escalate concerns appropriately
• Good interpersonal skills for comforting residents and collaborating with staff
