Location: Lebanon, NH

Contract Type: Per-Diem

Shift Type: Per diem / varied days, 3:00 PM - 11:00 PM, Evening shift

Hours per Week: 40

Candidate Pay Rate: $24 per hour

Roles & Responsibilities

Provide compassionate personal care and assist residents with activities of daily living
Monitor and document vital signs and patient observations accurately
Support mobility, transfers, and safe ambulation for residents
Assist the nursing team with care plans and follow assigned tasks promptly
Maintain a clean, safe environment and follow infection control practices
Communicate changes in condition or concerns to the charge nurse in a timely way
Build rapport with residents and families to enhance comfort and trust
Accurately complete required documentation and shift handoffs

Qualifications

Current Licensed Nursing Assistant (LNA) certification in good standing
Valid CPR/BLS certification preferred or willingness to obtain
Demonstrated ability to work evening shifts and adapt to per diem scheduling
Strong communication skills and a positive, team-focused attitude
Previous experience in long-term care or similar healthcare settings is a plus

Education

Completion of a state-approved LNA training program or equivalent
High school diploma or GED preferred

Knowledge & Skills

Skilled in personal care, bathing, dressing, and grooming assistance
Able to take and record vital signs and report changes
Knowledge of infection control and basic safety practices
Familiarity with patient transfer techniques and use of assistive devices
Good documentation habits and attention to detail
Compassionate approach to resident-centered care and teamwork
