Roles & Responsibilities
- Provide compassionate personal care and assistance with activities of daily living
- Support residents with mobility, transfers, and safe ambulation
- Observe and report changes in resident condition to nursing staff promptly
- Assist with feeding, toileting, grooming, and hygiene while preserving dignity
- Document care accurately and timely in the facility charting system
- Follow infection control practices and facility safety protocols
- Collaborate with the interdisciplinary team to deliver person-centered care
Qualifications
- Valid Licensed Nursing Assistant (LNA) certification for VT/NH
- Reliable transportation — local travel to the facility is required
- Weekend availability — every other weekend coverage expected
- Ability to pass facility background checks and any required screenings
- Strong communication skills and a team-oriented attitude
Knowledge & Skills
- Patient handling and safe transfer techniques
- Basic charting and documentation practices
- Infection prevention and standard precautions
- Ability to support activities of daily living (ADLs)
- Effective communication with residents, families, and care teams
- Time management and prioritization in a busy care environment
