Job Overview
We are seeking an enthusiastic COTA to join a Skilled Nursing Facility in San Jose, CA for a 13-week contract. Join a collaborative rehab team focused on improving resident outcomes and fostering independence.
Responsibilities

Carry out OT-designed treatment plans

Provide therapeutic activities and ADL training

Complete accurate care documentation

Communicate progress to supervising OT and care team

Participate in patient-centered planning & care goals
Qualifications

Active California COTA License
-
Previous SNF experience preferred

Strong teamwork & patient-care approach
