Roles & Responsibilities

• Deliver therapeutic procedures under the supervising therapist’s plan of care to maximize resident independence
• Implement and progress individualized treatment plans toward functional goals
• Document all therapeutic interactions timely and accurately so the whole care team stays informed
• Collaborate with residents, families, and the interdisciplinary team to ensure resident-centered care
• Recommend and instruct on appropriate assistive devices and ensure safe use
• Participate in team meetings and contribute to new program development in Rehabilitation
• Maintain clinical area cleanliness and follow infection control procedures to keep residents safe
• Recognize changes in resident status and promptly communicate with the evaluating therapist

Qualifications

• Associate degree required
• PT Assistant License required (state licensure)
• Basic Life Support (BLS) certification required
• Prior experience in long-term care or skilled nursing facility preferred
• Able to perform physical demands of the role, including lifting up to 50 pounds and frequent lifting/carrying up to 20 pounds
• Strong English communication skills, both written and verbal
• Willingness to maintain required licenses, certifications, and annual compliance training

• Associate degree (required)

Knowledge & Skills

• Therapeutic modalities and treatment planning
• Patient documentation and excellent charting skills
• Patient education and clear teaching strategies
• Interdisciplinary team collaboration and strong interpersonal skills
• Time management and ability to prioritize caseload
• Infection control procedures and safe patient handling
• Familiarity with Medicare/Medicaid reimbursement requirements
• Comfortable using Microsoft Office for documentation and communication
• Ability to assess need for assistive devices and provide instruction
