Physical Therapist - SNF
Location: San Francisco, CA
Start Date: 03-03-2026
Contract Type: Contract
Shift Type: Part-time (20 hours/week), schedule varies/PRN, exact days and times to be determined
Hours per Week: 20
Candidate Pay Rate: $150 - $105 per hour

Roles & Responsibilities
- Evaluate patients and create individualized physical therapy treatment plans that promote mobility and independence
- Deliver targeted therapeutic exercises, gait training, and adaptive strategies to improve function
- Monitor patient progress, adjust interventions as needed, and celebrate functional gains
- Collaborate closely with physicians, nursing, and rehab staff to coordinate patient-centered care
- Maintain accurate, timely clinical documentation to support continuity of care and outcomes
- Educate patients and families on home programs and safe mobility techniques
Qualifications
- Active California Physical Therapist License required
- Minimum 1 year of Physical Therapy experience
- Previous SNF or rehabilitation experience preferred
- Current BLS certification required
- Strong clinical assessment skills and a passion for geriatric care
Knowledge & Skills
- Skilled in clinical assessment, therapeutic exercise, and gait training
- Experience with care planning and progress documentation
- Ability to collaborate with interdisciplinary teams and communicate effectively
- Knowledge of mobility, balance, strength training, and functional independence strategies
- Comfort working in a skilled nursing facility environment and supporting geriatric patients
