Shift Type: Monday-Friday, 8:00 AM - 4:30 PM day shift

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Evaluate and treat residents to improve their mobility and independence
Develop personalized treatment plans that help patients reach their goals
Collaborate with the interdisciplinary team for holistic patient care
Document progress accurately to track successes and adjustments
Participate in care conferences to drive quality improvements

Qualifications

Active California Physical Therapist license required
Skilled nursing or post-acute experience preferred
Strong clinical and communication skills
Dependable, flexible, and team-oriented mindset
Commitment to compassionate, high-quality care

Education

Knowledge & Skills

Expertise in rehab therapy for skilled nursing settings
Ability to create effective individualized treatment plans
Strong interdisciplinary collaboration skills
Precise documentation following regulatory standards
