Roles & Responsibilities

• Provide skilled physical therapy assessment and treatment to residents, focusing on safe functional gains
• Develop individualized treatment plans that target mobility, strength, balance, and independence
• Document patient evaluations, progress notes, and discharge planning clearly and on time
• Collaborate with nurses, therapists, and care teams to coordinate patient-centered care
• Educate patients and families on home exercise programs and safe transfer techniques
• Monitor and report changes in patient condition, adjusting care plans as needed
• Maintain compliance with facility policies, safety protocols, and regulatory requirements

Qualifications

• Current, active Physical Therapist license in California (required)
• CPR/BLS certification (or willingness to obtain)
• Experience in skilled nursing or rehabilitation settings preferred
• Strong clinical assessment, treatment planning, and documentation skills
• Excellent communication and teamwork abilities to work across disciplines
• Ability to work the posted schedule: Monday–Friday daytime shifts

Education

• Doctor of Physical Therapy (DPT) preferred, or Bachelor's/Master's in Physical Therapy
• Required: Valid California Physical Therapy license

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, and mobility interventions
• Knowledge of manual therapy and fall prevention strategies
• Familiarity with electronic medical records and timely documentation
• Understanding of infection control and patient safety best practices
• Ability to educate patients and caregivers clearly and compassionately
