Roles & Responsibilities

• Provide high-quality, patient-centered physical therapy evaluations and treatments
• Develop and implement individualized treatment plans to help patients regain function
• Document patient progress clearly and timely in the medical record
• Collaborate with nurses, physicians, and therapy team members to optimize outcomes
• Educate patients and families on home programs, safety, and discharge planning
• Participate in team meetings and contribute to continuous improvement efforts

Qualifications

• Licensed Physical Therapist in California or eligible to obtain California licensure
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working in a fast-paced contract/travel setting and adapting to facility workflows
• Strong communication and interpersonal skills with patients and interdisciplinary teams
• Prior experience in inpatient/rehab or contract assignments is a plus

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s degree in Physical Therapy considered

Knowledge & Skills

• Solid assessment skills and ability to create individualized treatment plans
• Experience with therapeutic exercise, gait training, balance and mobility interventions
• Proficiency documenting in electronic medical records (EMR) and timely charting
• Good clinical reasoning, time management, and patient education abilities
• Ability to collaborate effectively with a multidisciplinary care team
