Roles & Responsibilities

• Evaluate and treat patients following physician orders and referrals
• Develop and update individualized treatment and discharge plans that truly change lives
• Document services accurately and on time to keep care coordinated
• Supervise COTAs in line with practice acts and facility policies
• Collaborate with interdisciplinary care teams and family members to build the best outcomes
• Maintain full compliance with safety and regulatory standards
• Help patients rebuild independence in their daily living skills

Qualifications

• Graduate of an accredited Occupational Therapy program (MOT or OTD)
• Active state Occupational Therapy license, or the ability to obtain one
• Strong clinical, documentation, and communication skills
• A genuine passion for patient-centered rehabilitation care
• Comfort working with an interdisciplinary team in a skilled nursing setting

Knowledge & Skills

• Occupational Therapy practice across a rehab and skilled nursing setting
• Patient evaluation and assessment techniques
• Treatment plan development and discharge planning
• Clinical documentation that is clear, timely, and accurate
• COTA supervision and mentorship
• Interdisciplinary collaboration and family communication
• Regulatory compliance and safety standards
