Roles & Responsibilities

• Deliver skilled physical therapy evaluations and treatments that help residents regain strength, mobility and confidence
• Build personalized care plans that reflect each resident's goals, abilities and daily routine
• Collaborate with nurses, CNAs, occupational therapists and physicians to keep every care plan moving forward
• Document progress clearly and on time so the whole care team stays on the same page
• Educate residents and families on exercises, safety and home routines they can carry forward
• Mentor therapy assistants and students while championing a culture of quality and compassion
• Celebrate the small wins, because every step forward matters

Qualifications

• Active Massachusetts Physical Therapist license (or compact eligibility)
• Graduate of an accredited Physical Therapy program
• Current BLS certification
• Experience in skilled nursing, long term care, or rehabilitation is a big plus
• New grads with a passion for geriatric care are welcome to apply
• Strong communication skills and a genuine love for helping people move better

Knowledge & Skills

• Hands on manual therapy and therapeutic exercise techniques
• Fall prevention, gait training and functional mobility strategies
• Familiarity with EMR documentation and therapy software
• Team based communication with interdisciplinary partners
• Patience, empathy and a positive attitude that lifts a room
• Time management that keeps treatments flowing without rushing care
