Roles & Responsibilities

• Evaluate and treat patients according to physician orders and referrals
• Create and update individualized treatment and discharge plans that help patients regain independence
• Document services accurately and on time to keep the care team informed
• Supervise COTAs in line with practice acts and facility policies
• Collaborate with interdisciplinary care teams and families to deliver coordinated care
• Maintain compliance with safety and regulatory standards to ensure high-quality services

Qualifications

• Graduate of an accredited Occupational Therapy program (MOT or OTD)
• Active state OT license or ability to obtain one
• Strong clinical, documentation, and communication skills
• Ability to work collaboratively in a skilled nursing setting

• Occupational therapy evaluation and treatment
• Patient assessment and individualized treatment planning
• Discharge planning and care coordination
• Clinical documentation and regulatory compliance
• COTA supervision and practice act knowledge
• Interdisciplinary collaboration and family communication
