Roles & Responsibilities

• Deliver compassionate, patient-centered care to residents
• Administer medications and treatments as prescribed
• Monitor vital signs and report changes to the healthcare team
• Assist with daily living activities and promote independence
• Collaborate with nurses, physicians, and therapists to ensure optimal outcomes
• Maintain accurate patient records and documentation

Qualifications

• Current LPN license in the state of New Hampshire
• BLS certification (American Heart Association)
• Ability to work evening shifts and commit to a 6-week contract
• COVID-19 vaccination or authorized medical/religious exemption

Knowledge & Skills

• Patient care and clinical assessment
• Medication administration and management
• EMR/EHR proficiency
• Strong communication and teamwork skills
• Adaptability in a fast-paced environment
