Roles & Responsibilities

Deliver compassionate, patient-centered care to residents
Administer medications and treatments as prescribed
Monitor vital signs and report changes to the healthcare team
Assist with daily living activities and promote independence
Collaborate with nurses, physicians, and therapists to ensure optimal outcomes
Maintain accurate patient records and documentation

Qualifications

Current LPN license in the state of New Hampshire
BLS certification (American Heart Association)
Ability to work evening shifts and commit to a 6-week contract
COVID-19 vaccination or authorized medical/religious exemption

Knowledge & Skills

Patient care and clinical assessment
Medication administration and management
EMR/EHR proficiency
Strong communication and teamwork skills
Adaptability in a fast-paced environment
