Candidate Pay Rate: $84.15 - $63 per hour

Roles & Responsibilities

• Administer medications and provide wound care, trach care, and IV therapy
• Monitor and assess patients with pulmonary diagnoses, Alzheimer's, and dementia
• Collaborate with an interdisciplinary team including PT, OT, SLP, and case managers
• Attend team conferences to coordinate care
• Provide compassionate, patient-centered care in a skilled nursing unit
• Document patient information using Cerner EMR

Qualifications

• Current California RN license
• BLS certification required
• Prior SNF or subacute experience strongly preferred
• Experience with Cerner charting system is a plus

• Skilled nursing and subacute care
• Wound care, trach care, and IV therapy
• Pulmonary and oxygen therapy management
• Compassionate communication with dementia patients
