Candidate Pay Rate: $29 per hour

Roles & Responsibilities

Provide compassionate, hands-on patient care consistent with LPN scope of practice
Administer medications and document treatments accurately and timely
Monitor patient status, record vital signs, and report changes to the care team
Collaborate with nurses and interdisciplinary staff to coordinate excellent care
Educate patients and families about care plans and discharge instructions
Maintain clean and safe care environments and adhere to infection-control protocols
Document assessments and care thoroughly in the patient record

Qualifications

Active Licensed Practical Nurse (LPN) license (required)
Current BLS/CPR certification (or obtain upon hire)
Clinical experience in a long-term care or skilled nursing setting preferred
Ability to pass background check, drug screen, and meet facility immunization requirements
Strong communication skills and a team-oriented mindset

Education

Completion of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and IV site observations
Patient assessment and vital sign monitoring
Electronic health record (EHR) documentation
Time management, prioritization, and effective patient communication
