Roles & Responsibilities
- Provide compassionate, hands-on patient care consistent with LPN scope of practice
- Administer medications and document treatments accurately and timely
- Monitor patient status, record vital signs, and report changes to the care team
- Collaborate with nurses and interdisciplinary staff to coordinate excellent care
- Educate patients and families about care plans and discharge instructions
- Maintain clean and safe care environments and adhere to infection-control protocols
- Document assessments and care thoroughly in the patient record
Qualifications
- Active Licensed Practical Nurse (LPN) license (required)
- Current BLS/CPR certification (or obtain upon hire)
- Clinical experience in a long-term care or skilled nursing setting preferred
- Ability to pass background check, drug screen, and meet facility immunization requirements
- Strong communication skills and a team-oriented mindset
Education
- Completion of an accredited Practical Nursing (LPN/LVN) program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and IV site observations
- Patient assessment and vital sign monitoring
- Electronic health record (EHR) documentation
- Time management, prioritization, and effective patient communication
