Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and on schedule
• Perform thorough patient assessments and promptly document findings
• Collaborate with the interdisciplinary team to create and follow care plans
• Monitor vital signs, assist with activities of daily living, and report changes
• Educate patients and families with clarity and empathy
• Maintain accurate, timely electronic health record documentation

Qualifications

• Active Licensed Practical Nurse (LPN) license (New Hampshire preferred or compact where applicable)
• Current BLS/CPR certification or willingness to obtain prior to start
• Prior LPN clinical experience in long-term care, rehab, or similar settings preferred
• Ability to pass background check and drug screen as required by facility
• Strong communication skills and a collaborative approach to patient care

Education

• LPN diploma or certificate (required)
• Active state LPN license (required)

Knowledge & Skills

• Proficient in medication administration and basic clinical procedures
• Skilled in patient assessment and recognizing changes in condition
• Comfortable with electronic health record documentation
• Good time management and prioritization during busy night shifts
• Infection control practices and safe patient handling
• Teamwork, clear communication, and compassionate bedside manner
