Roles & Responsibilities

• Assist residents with activities of daily living including bathing, dressing, grooming, and toileting with dignity and compassion
• Support the interdisciplinary rehab team by following care plans and reporting changes in condition promptly
• Safely transfer and ambulate residents, using proper body mechanics and assistive devices
• Monitor and record basic vital signs and intake/output as directed
• Maintain a clean, safe environment and adhere to infection control practices
• Document care accurately and communicate effectively with nurses, therapists, and families
• Provide emotional support and foster a respectful, person-centered atmosphere for residents

Qualifications

• GNA / Geriatric Nursing Assistant certification (required)
• Current CPR/BLS certification preferred or ability to obtain promptly
• Ability to work night shifts (11:00 PM - 7:00 AM) and adapt to per-diem scheduling
• Up-to-date immunizations and ability to pass required background checks
• Strong communication skills and a compassionate, patient-centered approach
• Previous experience in long-term care or rehab settings is a plus

Education

• High school diploma or GED
• Completed GNA / Geriatric Nursing Assistant training program

Knowledge & Skills

• Knowledge of activities of daily living (ADLs) and basic patient care
• Skilled in safe transfers, proper body mechanics, and use of assistive devices
• Good observational skills to identify changes in resident condition
• Accurate documentation and charting abilities
• Teamwork and collaboration with nursing and therapy staff
• Time management and ability to prioritize resident needs
