Roles & Responsibilities

Provide comprehensive outpatient family medicine care across the lifespan, from pediatrics to geriatrics
Build lasting relationships with patients and families through meaningful, patient-centered care
Manage chronic and acute conditions with thoughtful, evidence-based treatment plans
Collaborate closely with clinical and administrative staff to ensure coordinated, high-quality care
Participate in community health initiatives to directly impact the wellbeing of a rural population
Maintain required documentation and professional credentials to meet hospital and regulatory standards

Qualifications

MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for medical licensure in New York State
DEA certification or eligibility
Passion for community-focused, patient-centered primary care

Education

MD or DO from an accredited medical school

Knowledge & Skills

Strong foundation in family medicine and primary care principles
Experience or comfort providing outpatient-focused care across all ages
Excellent patient communication and relationship-building skills
Ability to work collaboratively within a multidisciplinary team
Commitment to rural and community health
