Contract Type: Perm

Shift Type: Monday-Friday, Day shift (hours discussed at interview)

Hours per Week: 40

Bill Rate: $24.77 - $30.11+ per hour (based on role and experience)

Benefits

Hourly Range: Care Manager $30.11+ / Social Worker BSW: $24.77+ (based on education and experience)
Relocation Allowance: Up to $5,000
Permanent Placement Fee: 18%
Admin fee 15% running through calendar year 2025

Roles & Responsibilities

Coordinate the plan of care to ensure quality, timely delivery, and medical necessity for each patient
Assess, plan, implement, monitor, and evaluate individualized care options and services
Collaborate with patients, families, and healthcare teams to achieve positive health outcomes
Advocate for patients to ensure their needs are met with compassion and respect
Facilitate seamless transitions of care across settings

Qualifications

Current Kansas RN license (or LBSW license for social worker track)
Associate's Degree in Nursing or Diploma (BSN preferred) OR Bachelor's Degree in Social Services
Two years of acute care med/surg nursing experience for RN track (or hospital social work experience preferred for social worker track)

Education

Associate's Degree in Nursing or Diploma from an accredited registered nurse program
Bachelor's Degree in Social Services

Knowledge & Skills

Expertise in care coordination and patient assessment
Strong care planning skills
Experience in acute care and med/surg nursing
Knowledge of social work principles and hospital social work
