Roles & Responsibilities

• Provide compassionate nursing care to residents in a long-term care setting
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and report changes to the healthcare team
• Assist with daily living activities and promote patient independence
• Collaborate with interdisciplinary teams to develop and implement care plans
• Document patient care accurately and maintain up-to-date records
• Educate patients and families on health management and wellness

Qualifications

• Active LPN license in Vermont or compact state
• Minimum 1 year of experience as an LPN in a skilled nursing or long-term care facility
• Current BLS certification (American Heart Association)
• Ability to work every weekend as required
• Strong communication and teamwork skills
• Must have reliable transportation (limited public transit in the area)

Education

• Graduate of an accredited practical nursing program

Knowledge & Skills

• Proficient in medication administration and patient assessment
• Knowledge of infection control practices and safety protocols
• Experience with electronic health records (EHR) systems
• Ability to prioritize tasks and work efficiently in a fast-paced environment
• Compassionate and patient-centered approach to care
