Roles & Responsibilities

• Coordinate and manage patient care from admission through discharge to ensure safe, timely transitions
• Conduct utilization review and admission reviews using InterQual and evidence-based guidelines
• Develop and implement individualized discharge plans and arrange DME, SNF, and ambulance transport as needed
• Collaborate with interdisciplinary teams, lead family meetings, and contribute to patient treatment plans
• Perform prior authorization work and manage appeals and denials with payers
• Document thoroughly and accurately in Epic and other computerized systems
• Assess needs across the lifespan—from neonates to geriatrics—and coordinate appropriate community resources

Qualifications

• California state license required; pending license accepted for screening but must be licensed by start date
• Minimum 2 years of acute care case management or discharge planning experience
• Travel experience required; first-time travelers may be screened
• Proficiency with Epic and computerized documentation systems
• Experience using InterQual criteria and determining medical necessity preferred
• Strong communication, organizational, and care coordination skills
• Familiarity with Medicare/MediCAL, HMOs, IPAs, and worker's compensation case management is a plus

Education

• Not specified in the posting; relevant clinical or case management degree preferred (for example, BSN, BSW, or Bachelor's in a related field)

Knowledge & Skills

• Knowledge of admission criteria, utilization review, and medical necessity guidelines
• Skilled in discharge planning, care coordination, and arranging post-acute care (SNF, DME)
• Experience with appeals, denials, benefits eligibility, and DRG/CMS considerations
• Comfortable working with interdisciplinary teams including PT, RT, pharmacy, social services, and rapid response
• HIPAA compliance and strong documentation practices
• Ability to organize and participate in family meetings and treatment plan development
