Roles & Responsibilities
- Perform comprehensive patient evaluations to assess functional performance and rehabilitation needs
- Develop and implement personalized treatment plans aligned with patient goals
- Deliver therapeutic activities, exercises, and adaptive strategies to maximize independence
- Monitor and document patient progress while ensuring compliance with state and federal guidelines
- Educate patients and families on therapy goals, adaptive equipment, and home programs
- Collaborate with PTs, nurses, physicians, speech therapists and the interdisciplinary team to coordinate excellent care
Qualifications
- Active and current Occupational Therapist (OT) license in the state of practice (required)
- Graduate of an accredited Occupational Therapy program (OTD or equivalent)
- Previous experience working in a Skilled Nursing Facility (SNF) is required
- Strong clinical knowledge with a patient-centered approach
- Excellent communication, organization, and teamwork skills
Education
- OTD or equivalent — graduate of an accredited Occupational Therapy program

- Expertise in therapeutic activities and adaptive strategies
- Clear, compassionate patient and family education
- Strong teamwork and interdisciplinary collaboration
- Effective organization and clinical documentation practices
