Location: Rancho Cordova, California

Contract Type: Contract
Shift Type: 12:00 PM - 8:30 PM, every other weekend, rotating weekends about every 4 weeks
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Deliver compassionate hospice care to patients in homes and facilities across the greater Sacramento area
- Manage a caseload of 12-14 patients with 4 visits or 2 start-of-care assessments daily
- Collaborate with interdisciplinary teams to provide holistic end-of-life support
- Utilize Homecare Homebase (HCHB) for accurate documentation and care coordination
- Make a profound difference in the lives of hospice patients and their families every day
Qualifications
- Active RN license in California
- BLS (AHA) certification
- 2 years of nursing experience including at least 1 year in hospice
- 2 supervisory references: one within 12 months and one within 3 years from hospice setting
Education
- BSN preferred but not required
Knowledge & Skills
- Hospice patient care expertise
- Home care and facility-based service delivery
- Proficiency in Homecare Homebase (HCHB) EMR
- Strong assessment and visit management skills
