Roles & Responsibilities
- Provide compassionate psychosocial assessments to support patient well-being
- Develop meaningful care plans that make a real difference in lives
- Collaborate with multidisciplinary teams to coordinate seamless discharges
- Advocate fiercely for patients and families navigating complex healthcare needs
- Facilitate crisis interventions with empathy and expertise

Qualifications
- Active Licensed Clinical Social Worker (LCSW) License
- BLS (AHA) certification
- Minimum 2 years of relevant experience
- Experience in acute care settings preferred
Education
- Master's degree in Social Work or related field
Knowledge & Skills
- Expertise in social work practices
- Strong crisis intervention and assessment skills
- Proficiency with Cerner charting system
- Excellent communication and advocacy abilities
