Roles & Responsibilities

• Provide compassionate direct patient care, including administering medications, wound care, and daily living assistance
• Assess and monitor patients' vital signs and condition, reporting changes to the RN or healthcare team
• Document patient care and observations accurately in accordance with facility policies
• Collaborate with nurses, doctors, and other professionals to implement and update individualized care plans
• Educate patients and families on care plans, medications, and healthy practices
• Maintain a safe, clean, and supportive environment that promotes patient dignity and well-being

Qualifications

• Active Licensed Practical Nurse (LPN) license in West Virginia or a compact state
• BLS certification required (American Heart Association)
• Previous experience in long-term care or skilled nursing preferred
• Completion of COVID-19 vaccine or documented exemption per facility policy
• Ability to pass background check and pre-employment screenings

Knowledge & Skills

• Solid understanding of LPN scope of practice and patient safety standards
• Strong clinical skills including medication administration, phlebotomy, and wound care
• Effective communication and teamwork abilities with patients, families, and staff
• Compassionate, patient-focused approach with attention to detail
• Flexibility to adapt to varying shift schedules and resident needs
