Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (day shift), guaranteed 40 hours/week; Emergency Department coverage as needed 10:00 AM - 8:00 PM; occasional weekend coverage approximately every 3-4 weeks (EOW rotation possible)

Roles & Responsibilities

• Conduct comprehensive social work assessments for inpatient and acute care patients
• Perform mental health assessments and provide appropriate interventions or referrals
• Coordinate discharge planning to ensure safe transitions and appropriate follow-up care
• Prepare and file Title 36 petitions as indicated and manage related documentation
• Address social determinants of health (SDOH) through consults and care coordination
• Support maternal-child needs through maternal and child assessments in the Family Birthing Center
• Collaborate with interdisciplinary teams in ICU and Emergency Department settings
• Complete DCS and APS reporting and other mandated reporting accurately and timely
• Manage a caseload (typically 12-16 consults per day) while maintaining clear documentation in Cerner

Qualifications

• LMSW or LCSW required (license in good standing)
• Minimum 1 year of social work experience (acute care or hospital experience preferred)
• Experience with Cerner EMR strongly preferred
• Comfort working across settings including inpatient units, ED, ICU, and Family Birthing Center
• Strong assessment, documentation, and case management skills
• Ability to complete Title 36 petitions and mandated reporting (DCS/APS) when needed
• Willingness to participate in 1-2 week department-specific orientation with Social Work Manager

Education

• LMSW or LCSW required (license required)

Knowledge & Skills

• Clinical social work assessments and mental health evaluation techniques
• Discharge planning and care coordination best practices
• Case management for complex medical and psychosocial needs
• Familiarity with Title 36 processes and mandated reporting (DCS, APS)
• Understanding of SDOH factors and community resource navigation
• Experience charting and documenting in Cerner
• Ability to manage moderate caseloads (12-16 consults/day) with clear prioritization
