Roles & Responsibilities

• Provide high-quality physical therapy evaluation and treatment to patients, using evidence-based practices
• Develop individualized treatment plans and update goals in collaboration with the care team
• Document assessments, progress, and discharge information accurately and in a timely manner
• Communicate clearly with patients, families, and interdisciplinary staff to support continuity of care
• Monitor patient safety and modify interventions to meet patient needs and facility guidelines
• Participate in team meetings and contribute to problem-solving for complex patient cases

Qualifications

• Licensed Physical Therapist in California or actively pursuing California licensure
• Current basic life support (BLS) certification
• Clinical experience as a practicing Physical Therapist; facility experience such as medical, rehab, or correctional settings is a plus
• Strong documentation skills and ability to meet facility charting requirements
• Professional, dependable, and able to work Monday through Friday on a consistent schedule

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor or Master in related field with relevant clinical experience considered

Knowledge & Skills

• Skilled in musculoskeletal and functional mobility assessment
• Proficient in designing and progressing therapeutic exercise programs
• Familiarity with therapeutic modalities, gait training, and assistive device recommendations
• Solid clinical reasoning and problem-solving abilities
• Excellent verbal and written communication with patients and interdisciplinary teams
• Ability to follow facility protocols and maintain patient safety
