Roles & Responsibilities

• Provide comprehensive physical therapy evaluations and treatments for residents in a skilled nursing facility
• Develop and implement individualized care plans to improve mobility, strength, and functional independence
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Document patient progress and adjust treatment plans as needed
• Educate patients and their families on therapeutic exercises and fall prevention
• Maintain compliance with state practice guidelines and facility policies

Qualifications

• Active Physical Therapist (PT) license in the state of North Carolina
• New graduates are welcome to apply
• Experience in skilled nursing or geriatric rehabilitation is preferred but not required
• Strong clinical reasoning and a compassionate approach to patient care

Knowledge & Skills

• Solid understanding of SNF regulations and documentation standards
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a collaborative team
• Proficiency in developing and adjusting treatment plans based on patient progress
• Knowledge of Medicare and insurance billing for therapy services
