Roles & Responsibilities

• Provide comprehensive physical therapy evaluations and treatments to patients in the Hospital at Home program
• Develop individualized care plans to help patients regain mobility and independence
• Collaborate with the interdisciplinary care team to ensure seamless patient-centered care
• Document all patient interactions and progress accurately in Epic EMR
• Educate patients and families on therapeutic exercises and safety strategies

Qualifications

• Active BLS certification
• Current Physical Therapist license or eligibility for Michigan licensure
• EPIC EMR experience required
• Previous experience in home health or acute care preferred
• Ability to work Monday-Friday daytime schedule with no weekends or holidays

Knowledge & Skills

• Knowledge of evidence-based physical therapy practices
• Skill in using Epic EMR for documentation
• Strong communication and patient education skills
• Ability to work independently in a home health setting
• Competency in physical therapy specialty testing
