Roles & Responsibilities

• Provide compassionate, one-on-one physical therapy in patients' homes
• Develop personalized treatment plans to help patients regain independence
• Perform comprehensive assessments and adjust care to each patient's progress
• Educate families and caregivers on exercises and home safety
• Collaborate with healthcare teams to ensure seamless, coordinated care
• Document every visit with precision and empathy

Qualifications

• Active Physical Therapist (PT) license in Iowa (required)
• BLS certification (required)
• Home health or outpatient experience preferred
• Comfort with flexible scheduling, including weekends if needed

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Excellent communication to connect with patients and families
• Ability to work independently and manage a home health caseload
• Familiarity with Epic EMR is a plus, but not required
