Roles & Responsibilities

• Evaluate residents and create individualized occupational therapy treatment plans that promote independence
• Deliver focused therapy for ADLs, mobility, and functional skills to improve everyday living
• Implement therapeutic exercises and activities to enhance coordination and upper extremity function
• Monitor resident progress, adjust treatment plans as needed, and document outcomes accurately
• Collaborate with physicians, therapists, nursing, and interdisciplinary care teams to coordinate care
• Educate residents and caregivers on adaptive techniques and safety strategies to maximize independence
• Maintain timely and precise clinical documentation to support quality care and continuity

Qualifications

• Active California Occupational Therapist license (required)
• Degree in Occupational Therapy from an accredited program (required)
• Previous SNF, rehabilitation, or geriatric therapy experience (preferred)
• Strong clinical assessment and patient care skills with proven treatment planning ability
• Excellent communication and documentation skills and ability to work within a multidisciplinary team

Education

• Degree in Occupational Therapy from an accredited program

• Expertise in ADL training and functional rehabilitation
• Patient evaluation and treatment planning
• Clinical assessment and evidence-based intervention techniques
• Effective interpersonal communication with residents, families, and care teams
