Roles & Responsibilities

• Evaluate residents' physical and cognitive abilities to develop personalized treatment plans that maximize independence and quality of life.
• Provide therapeutic interventions focused on daily living skills, safety, and functional mobility in a skilled nursing setting.
• Collaborate with nurses, physicians, and the interdisciplinary team to ensure cohesive, patient-centered care.
• Educate residents and their families on adaptive strategies, assistive devices, and home modification recommendations.
• Maintain accurate documentation of evaluations, progress notes, and discharge plans in compliance with facility and regulatory standards.
• Mentor and support rehab aides and students while fostering a positive therapeutic environment.

Qualifications

• Valid Illinois Occupational Therapist (OT) license required (or ability to obtain before start).
• New graduates are encouraged to apply - we welcome all experience levels!
• Previous SNF or long-term care experience is helpful but not mandatory.
• Compassionate, adaptable, and team-oriented approach to care.
• Must pass background check and comply with facility vaccination requirements.

Knowledge & Skills

• Strong clinical reasoning and assessment skills.
• Knowledge of occupational therapy best practices in geriatric rehab.
• Proficiency with electronic medical records (EMR) - training available.
• Excellent communication and interpersonal skills.
• Ability to adapt treatment plans to meet diverse patient needs.
