Roles & Responsibilities

• Provide compassionate bedside care to patients in the Uptown Health/Rehab area
• Administer medications and treatments safely and accurately according to plan of care
• Perform focused patient assessments and monitor changes in condition
• Document patient status and interventions clearly and timely in the medical record
• Collaborate with nurses, therapists, and providers to deliver coordinated care
• Educate patients and families on care plans, discharge instructions, and safety
• Assist with admissions, transfers, and safe patient handling to ensure continuity of care

Qualifications

• Active Licensed Practical Nurse (LPN) license — New Mexico licensure required
• Graduation from an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification (current) — preferred or required by facility
• Prior LPN experience in long-term care, rehab, or acute settings preferred
• Strong communication skills and the ability to work well in a multidisciplinary team
• Local candidates only (facility notes indicate local-only applicants)

Education

• Completion of an accredited LPN/vocational nursing program (diploma or certificate)

Knowledge & Skills

• Skilled in medication administration and basic wound care
• Comfortable performing vital signs, patient assessments, and ADL assistance
• Proficient at clear documentation and following established care plans
• Ability to prioritize tasks in a fast-paced clinical environment
• Team-oriented mindset with good clinical judgment and infection control practices
