Roles & Responsibilities

• Provide direct, compassionate care to residents including assistance with activities of daily living
• Observe and report changes in patient condition to the nursing team promptly
• Assist with safe transfers, mobility support, and resident comfort measures
• Follow established care plans and document care accurately and timely
• Support the team during admissions, discharges, and shift handovers to ensure continuity of care

Qualifications

• Current Licensed Nursing Assistant (LNA) credential; T-LNA classification noted as relevant
• Weekend availability: every other weekend is required
• Reliable transportation to and from the facility
• Ability to begin assignment on 2026-04-14 for a 14-week contract through 2026-07-14
• Willingness to work scheduled day shifts and follow facility policies

Education

• Licensed Nursing Assistant (LNA) certification (T-LNA classification noted)

Knowledge & Skills

• Patient-centered care and strong interpersonal communication
• Familiarity with resident ADLs, mobility assistance, and safety protocols
• Basic clinical observation and timely reporting skills
• Dependability, teamwork, and ability to follow care plans and documentation standards
• Comfort working in long-term care and rehabilitation environments
