Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes across a 60-mile radius
- Help patients recover from surgeries, falls, and other conditions with personalized treatment plans
- Support both Home Health and Hospice patients on their wellness journeys
- Document care using HomeCare/HomeBase EMR system
- Build meaningful relationships while delivering therapy that truly matters
Qualifications
- Current North Dakota Physical Therapy License required
- BLS certification needed
- At least 1 year of physical therapy experience with previous home health background
- Valid driver's license and auto insurance essential
- Strong PT-related clinical skills
Knowledge & Skills
- Home health physical therapy expertise
- Experience with post-surgical and fall recovery patients
- Comfortable working independently in home settings
- Familiarity with HomeCare/HomeBase EMR preferred
- Excellent communication and time-management abilities
