Roles & Responsibilities

• Provide compassionate, professional nursing care to clients of all ages in the home setting
• Perform admissions, thorough evaluations, and develop individualized care plans
• Coordinate care with a team of homecare professionals and paraprofessionals to ensure continuity
• Educate patients and families on care plans, medications, and safety measures
• Document clinical findings, interventions, and outcomes accurately and timely
• Advocate for patients’ needs and collaborate with community resources to support care goals
• Monitor and report changes in patient condition, using clinical judgment to escalate when needed

Qualifications

• Current Vermont RN license (required)
• Active Registered Nurse with at least 1 year of prior clinical experience
• Comfortable providing care across the lifespan (experience with all ages preferred)
• Team-oriented mindset with strong communication and collaboration skills
• Willingness to work occasional weekends as needed
• Experience with hospice care is a plus
