Roles & Responsibilities

• Coordinate comprehensive care and discharge planning to ensure smooth, safe transitions home or to next level of care
• Use Epic to document care, track progress, and communicate with the interdisciplinary team
• Perform utilization review and apply InterQual criteria to support appropriate level-of-care decisions
• Arrange DME, SNF placement and ambulance transport independently to meet patient needs
• Lead or participate in family meetings and contribute to individualized treatment plans
• Collaborate with social services, therapy teams, and case partners to resolve barriers to discharge
• Manage prior authorizations and benefits eligibility to expedite patient care and reduce delays
• Maintain compliance with regulatory standards such as HIPAA, CMS, DRG processes and Joint Commission goals

Qualifications

• Minimum 2 years acute hospital case management experience performing care coordination and discharge planning
• Travel experience required; first-timers may be screened with clinical approval
• Charting experience in Epic (required)
• Comfort floating between like units based on department need
• Ability to manage a patient ratio up to 1:20
• BLS preferred
• Familiarity with InterQual criteria, prior authorizations, CPT/ICD-10 concepts, DRG and regulatory guidelines

Knowledge & Skills

• Case management and care coordination in acute hospital settings
• Discharge planning and needs assessment
• Utilization review using InterQual criteria
• Prior authorization processes and benefits eligibility
• Epic electronic charting and computerized documentation
• CPT and ICD-10 basics, DRG knowledge, HIPAA and CMS regulatory awareness
• Experience arranging DME, SNF and ambulance transport independently
• Strong interdisciplinary communication and family meeting facilitation
