Roles & Responsibilities

• Provide compassionate, hands-on personal care to residents, helping with activities of daily living like bathing, dressing, grooming, and toileting
• Take and record vital signs accurately and report any changes to the nursing team promptly
• Assist with safe transfers and mobility support, always prioritizing patient safety
• Document care activities clearly and timely in the resident chart or EMR
• Maintain infection control practices and a clean, comfortable environment for residents
• Communicate effectively with nurses, families, and the interdisciplinary team to support resident-centered care
• Support facility routines and participate in team huddles to help deliver consistent, high-quality care

Qualifications

• Active LNA certification (T-LNA classification noted) or equivalent required
• Current CPR/BLS certification preferred or willing to obtain
• Reliable transportation required (public transit is limited in the area)
• Background check and required immunizations up to date
• Previous experience in long-term care or similar settings is a plus
• Strong communication skills and a positive, team-oriented attitude

Education

• High school diploma or equivalent
• Current LNA certification required

Knowledge & Skills

• Comfortable assisting with activities of daily living and personal care
• Skilled at taking and recording vital signs accurately
• Familiarity with basic infection control principles and resident safety
• Clear, compassionate communication with residents, families, and staff
• Basic charting/EMR documentation skills and attention to detail
• Ability to work as part of an interdisciplinary care team
