Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Deliver personalized physical therapy care to patients in their homes across Tehama County
- Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
- Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
- Complete approximately 5 meaningful patient visits each day using your clinical expertise
- Document care accurately using the HomeCare HomeBase EMR system on your provided tablet
- Collaborate with the intake RN and manager to coordinate seamless patient care
Qualifications
- Active Physical Therapist license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience if they have Home Health background
- Comfortable traveling within the service area and managing a daily mileage of approximately 40 miles
Knowledge & Skills
- Physical Therapy assessment and treatment planning
- Home Health documentation and compliance
- Orthopedic and geriatric patient care expertise
- Strong communication and patient education skills
- Ability to work independently in home settings
