Roles & Responsibilities

• Assess speech, language, and swallowing disorders for residents in a post-acute care setting
• Develop and implement individualized treatment plans to improve communication and cognitive function
• Provide therapeutic interventions to assist residents with speech, language, and swallowing difficulties
• Collaborate with interdisciplinary care teams to ensure coordinated patient care
• Document progress accurately and adjust treatment plans as needed
• Educate patients and families on strategies for communication and dysphagia management

Qualifications

• Active Speech Language Pathologist license in the state of California
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
• Experience in post-acute or skilled nursing settings is a plus
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of a healthcare team
• Current CPR/BLS certification

Knowledge & Skills

• Knowledge of speech therapy techniques and modalities
• Understanding of medical terminology and documentation
• Proficiency in electronic health records
• Skills in patient assessment and treatment planning
• Ability to adapt to different patient needs
• Excellent organizational and time-management skills
