Roles & Responsibilities

• Welcome new patients into hospice care and complete comprehensive admissions assessments
• Partner with patients and families to create individualized, compassionate plans of care
• Coordinate with a multidisciplinary team to ensure seamless, holistic support
• Document observations and interventions accurately and thoroughly
• Participate in quality assessment and performance improvement initiatives to elevate care
• Conduct follow-up visits to support patients wherever they call home

Qualifications

• Active and clear California RN license
• Current BLS/BCLS certification
• Minimum of one year of hospice nursing experience
• A heart for end-of-life care and a calm, compassionate approach

Knowledge & Skills

• Strong patient assessment and critical thinking skills
• Expertise in hospice and palliative care principles
• Skilled in care planning and multidisciplinary coordination
• Proficient in accurate medical documentation
• Excellent communication and family support skills
